A new pilot program in Fairfield is offering a fresh, holistic approach to healthcare.

The Fairfield Social Prescribing Project aims to connect people with chronic health conditions, who are experiencing social isolation and loneliness, to local services and community-based activities that support their overall health.

The project is being trialed as part of the existing Keeping Well in Community Care Navigation and Care Coordination program, run by the South Western Sydney Local Health District, which supports people managing chronic conditions by helping them stay healthy and avoid unnecessary hospital visits.

A central feature of the Fairfield Social Prescribing Project is the role of a Link Worker, who works closely with the KWIC CNCC team and individuals to assess their health and social needs and co-create a tailored care plan. Over 12 weeks, participants are supported to address their health needs whilst being supported to connect with local groups, services, and resources aligned with their personal goals and interests.

The Fairfield Social Prescribing Project will continue through to March 2026.

Eligibility:

This free program is for:

  • Residents of the Fairfield Area
  • People aged 16 and over
  • People with chronic and complex health conditions
