Approved forms for cooling water systems

Under the Public Health Act 2012 the approved forms must be used as follows:

• Form 1 – Risk Management Plan
• Form 2 – Audit report
• Form 3 – Monthly report
• Form 4 – Notification of reportable test results
• Form 6 – Notification of installation and change in particulars
  (Note: For any new systems Councils Registration of Business Facility form must also be completed and submitted to Council.)
